Documentation Requirements for issuance of UCB Credit Card

  • Completed Credit Card Application Form
  • Recent two copies of PP size color photograph with clear visibility duly signed by the applicant
  • Supplementary applicant’s photograph & signature duly attested by the applicant (If applicable)
  • Authorization Letter for Bank Statement Verification duly signed by the applicant
  • Copy of Personal TIN
  • Copy of NID
  • Copy of Passport/Driving license/ Commissioner Certificate with photo (additional if required)
  • Business/Visiting Card
  • Utility bill copy (if applicable)
  • Completed KYC
  • CIB enquiry & CIB undertaking form

Account Payee Salaried
  • LOI on official letterhead with proper Salary break down (signed by Manager Branch/Accounts/HR/Admin/Finance) or Pay slip stating Company Name.
  • Office ID
  • Latest 3(three) months ‘Original bank statement for A/C Payee and Partial A/C Payee salaried executives

Cash Payee Salaried
  • LOI on official letterhead with proper Salary break down (signed by Manager Branch/Accounts/HR/Admin/Finance) or Pay slip stating Company Name.
  • Cash Voucher
  • Office ID
  • Latest 6(six) months ‘ Original bank statement
Proprietorship
  • Photocopy of latest trade license
  • Latest 12 months Personal or Business bank statement
Partnership
  • Photocopy of latest trade license
  • Latest 12 months Company Bank Statement
  • Partnership Deed (Registered/ Notarized)
Limited Company
  • Copy of Memorandum and Articles of Association
  • Copy of Certificate of Incorporation
  • Latest 12 months Company Bank Statement
Self-Employed Professionals (Doctors, Engineers etc)
  • Latest 6(six) months Personal bank statement.
  • Professional Certificate
  • Declared Income on personal letterhead
Landlord/ Landlady
  • Ownership Proof (i.e. Registered Deed/ Utility Bill/ Mutation Copy/ Duplicate Carbon Receipt/ Holding Tax etc.)
  • Rental Agreement (Should be on Government Revenue Stamp Paper)
  • Latest 6(six) months Personal bank statement.
ERQ Card
  • Board Resolution along with a lien request letter for issuance of ERQ
  • Confirmation of ERQ A/C Lien (for UCBL/other bank ERQ).
Lien Card
  • Common requirements for individual card.
  • Lien Confirmation.
