Documents Required

Mandatory for all

• Application form duly filled in and signed by the Applicant
• Copy of e-TIN certificate
• Copy of latest tax return acknowledgement receipt
• Copy of Applicant's NID
• One clear, recent passport size photo of the Applicant, signed by the Applicant and attested by Bank official
• Original bank statement: Salaried- 6 months*, Landlord- 6 months, Business & Self-employed- 12 months

Nominee Documents

• One clear, recent passport size photo attested by Applicant
• One copy of NID/Birth Certificate

Salaried

• Pay slip/Letter of Introduction (LOI), or Salary certificate on official letterhead (with proper salary breakdown)
• Cash voucher (cash portion must be mentioned in LOI/Payslip/Salary certificate)
• Copy of Employee ID card and Visiting card
• Contract agreement (required for contractual employee)

Self-employed

• Copy of professional certificate permitting practice from the appropriate authority (e.g. Doctor - BMDC, Lawyer- Bar Association, Chartered Accountant- ICAB, Freelancer- Government-issued freelancer ID)
• Declaration of income on chamber/business letterhead pad

Landlord/ lady

• Ownership proof document (Title Deed/ Mutation/ Holding Tax copy)
• Copy of valid rental agreement deed (on revenue stamp)/rental receipt
• Any utility bill copy
• Cheque(s) amounting to 120% of credit card limit

Business

Mandatory for all business person

• Business card/Visiting card
• Trade license (last three years’ with updated one)
• Undated Cheque(s) equivalent to 120% of the approved/requested credit card limit

For Limited Company

• MOA, Form-X, Form-XII or Form 115-117
• Certificate of Incorporation and Certificate of Registration of the firm

For Partnership

Valid/Registered partnership deed

Secured Card

• Lien authorization letter
• FD Advice/DPS receipt 
• Board Resolution along with a lien request letter (ERQ Card)

Supporting documents to verify additional income other than your salary/business income.
